We kindly request that appointments be canceled or rescheduled at least 24 hours in advance to avoid a $20 cancellation/rescheduling fee.
Failure to attend an appointment without prior notice or cancellation after the specified time frame will result in a penalty fee of $89.
Following three consecutive no-shows, we reserve the right to suspend your appointment scheduling privileges.
All refill requests must receive authorization from your doctor. Prior to refills, physical exams may be required at the discretion of the doctor, taking into consideration patient history. Please allow 48 hours for medication refills to be processed. Certain medications have short expiration dates, so kindly request refills 15 days before pickup to ensure availability.
Patients with chronic conditions necessitate periodic examinations and blood work, determined at the discretion of the doctor based on individual needs.
There is a nominal fee of $15 for providing or sending a prescription to you for use at other pharmacies.
Payment is due at the time of service. A 50% deposit is mandatory for surgeries and ultrasounds to secure the scheduled date. Examinations typically run for 20-30 minutes; any additional time will incur an extra charge.
